Hospital Discharge Support

Coming home from the hospital can be a complicated and confusing process. A Vivia Assistant can provide patients a ride home at discharge and assist them with getting settled.  Services can include:

• Pick up of medications from the pharmacy
• Pick up of a prepared meal
• Assist with toileting at home
• Assist with changing of clothes at home
• Other tasks as requested and mutually agreed upon

Your Vivia Assistant can even provide quick assessment of wellness for vulnerable patients who are at high-risk for hospital readmission or ED visits.
